I would like to pursue EM training after IM (for numerous reasons)...is it possible to go this route without repeating the intern year?

I am a PGY-2 and would like to transition to EM in 2011-2012 preferably as a PGY-2. Anyone here gone this route? Suggestions for smoothing the transition (i.e. contacting programs ASAP?!?)

I know someone who is doing EM after IM. If you go to a PGY2-4 program, then obviously you start as a PGY-2. If you go through a traditional (PGY1-3 or 1-4) program, then the most credit you can get for your previous residency is 6 months (as per RRC guidelines).
 
Can you even do EM after IM? I understand there is a funding issue, and you are banned. How are you going to get around the funding issue? Are you going to work for no pay? I'd like to know.

You are never "banned" from doing any residency. And you wouldn't be working without any funding, it would be 50% of total yearly funding.

getting a PGY-2 spot in a 1-4 program ain't so easy, however.

Are you sure about this? Maybe you're right, but it goes against what I've been told by quite a few people.

Yes, I am sure.
The six months of credit rule has nothing to do with not doing the first year of a 1-4 program. All that matters is 36 months in an EM residency. Not doing that year in a 1-4 program is up to each PD...again, hard to find a 1-4 program with an open PGY-2 spot.

I thought when a program was approved for a 48 month EM training program, they cannot graduate anyone who only trains for 36 months in the program?

At any rate, I think the OP should ask a residency program coordinator and not an internet discussion forum.
 
That's the next step...just wanted some background information before calling.
 
What about the opposite switch? A good friend of mine is in EM training, but is now wondering whether primary care IM or FP would be a better fit. Would they be able to start as a PGY-2 if they transferred?

Where can I find out about the 6-month rule? I heard that you had to spend 36 months in an EM residency to sit for boards.
 
Every program I have interviewed at has had people switch back and forth generally between EM, IM, and occasionally FP. Go talk to your PD.
