Just wanted to get some opinions from the crowd. I'm a mid-career EM doc with some very significant family reasons for needing to return to NYC. I've been looking at jobs, and they all look pretty poorly paid and very high on minimum hours, and they all appear to pay annually, not hourly, which always seems to be a bad gig. There appear to be a ton of open positions, yet nothing seems negotiable, and there's little salary transparency.

Does anyone live in NYC and do solely locums? How much do per diem gigs in NYC pay? Are there any jobs in NYC that pay hourly? Any suggestions on the best way to engineer a move back to NYC for a mid-career EM doc?

Any thoughts/help would be appreciated...

I've heard that LIJ is a nice system to work for. Have you looked there?

I have some friends who have recently gotten jobs in the Hudson valley area that appear to pay well. May be worth the commute.

Your experience seems pretty typical for NYC.

If by locums you mean like far away locums, I have heard of people living in NYC and doing solely that. They are typically away for a significant stretch of time though, like almost two weeks a month. I do know some folks who live in NYC and work a little out of the city, like somewhere New Jersey, out on Long Island, or somewhere north of the city (it's all Upstate to me at that point). Also there are folks who have taken these "firefighting" gigs with some CMGs like TeamHealth and they get thrown around to some places generally within driving distance. They get paid a bit more for their trouble, but based on their description of what that's like I would not necessarily recommend that.

I've only seen per diem people get paid hourly. The per diem rate is probably going to be in the $180-250/hour range, depending on the institution, time of day, etc. I think the median is closer to the $200 mark. They will also probably want a 3-4 shift/month commitment.

Thanks! Super helpful replies from both of you!

Gonnabe- do you know where in the Hudson Valley?

Gro- appreciate the info on per diem. Looks like maybe the best combo would be per diem in NYC and some locums elsewhere. I'm just not seeing the point of a 1716 hour a year job in a dysfunctional ED that pays 300k a year with no overtime, which seems to be about the best gig available in NYC, although if anyone can enlighten me...

Thanks again for the replies. Love to hear from any other docs that would care to chime in.
