Do Em Physicians have repeating monthly schedules and/or the ability to request specific days off in advance? Or is it more of a "get what you get" sort of deal?

Varies widely. Everything from block schedules to request based to "you'll take what you get and like it". It takes a large number of physicians per shift to consistently do some of the fancier things (advancing circadian schedule, etc) although if everyone in the group prioritizes schedule and work happiness over everything else in their lives than you could do it with far fewer docs. Absent a huge number of docs per shift, it tends to be" you can make up to x requests/month" and expect them to be honored The scheduler does their best to create something that isn't horrible that still accommodates everyone's requests. A lazy scheduler screws everybody although groups will that know their scheduler sucks will often trade amongst themselves to improve things. A scheduler that plays favorites can easily wreck a group.
