Hey guys, I need advice.

I know its pretty common for a temporary crown to fall out (although it should not), but how often is it that the core falls out together with the temporary crown? This happened to me and honestly I'm feeling genuinely bad for the patient because the e.max crown has already been fabricated yet she has to endure through the whole crown process again

I'm just feeling a tad frustrated because critically speaking a post should have been used but I have been advised that a core would have been enough 🙁

Thanks for any help in advance

If you did the core the same day as the prep and provisional, then it isn’t too surprising that that happened. You need to lube the prep a little before fabrication/cementation of the provisional or else they will bond (I’m not sure of the actual science behind that, but it used to happen to me all the time).
As for what to do now, as long as the margins aren’t affected and you can approximate the fit of the crown still, there may be hope. Just line the intaglio of the crown with PTFE (Teflon) tape, fill it with your core material (ideally a self/dual cure so you don’t have to try and light cure it too deep), approximate it onto the prep, wait for it to set up, pull it off, take out the tape from the intaglio and boom, now you have a retrofitted core and your crown will still work. I found a paper once backing this technique up that had a successful 5 year follow up. It was a case study so the evidence isn’t too strong, but since then, I have used it multiple times and have not had a problem yet.

Omg thank you so much, I actually feel so much better after what you just said.

I asked a senior and she did mention about me having to apply vaseline on the core before fabrication/ cementation of the provisional as well. Sigh I wished I knew about it earlier. Thank you so much for the tips!! I will suggest it to my tutor and hopefully something will work out otherwise I'll just have to redo the entire thing.

I’m pretty sure it happens to everyone at some point. Good luck!
