I am IMG with vast experience in Emergency medicine in my Country, have done pediatric residency in US and I am board certified in pediatrics. I have been offered a job in community hospital ER which sees both pediatric and Adult patients. Can I accept the job , will it be illegal to work in general ER. They are giving me good malpractice insurance and tail coverage. Please advise.

It's not "illegal", but you have to be credentialed, and it is highly unlikely that you will be credentialed by the hospital to see adults. If there is a dedicated peds side to the ED, it is highly likely that that will be your 100% job.
