I am joining a family med practice and am setting up the pediatric part of the practice.
I am looking for any good pediatric encounter forms. Initial patient history questionnaires, sick visits, well-child visits, etc. I know AAP has some that can be purchased, but I wanted to see what alternatives other peds clinics were using. Bright Futures had some good ones, but they are no longer in print & are out of date.
Any suggestions or examples that anyone wouldn't mind sharing??
