english requirement

It depends. That sounds a lot like how AP credits are handled at most institutions. Some medical schools don't accept credit like that for pre-reqs, some will require you to take more advanced courses in that area, and some don't care. You'll need to look at each school individually to figure out what they require.

Pretty sure APs are fine and extension programs that show up on transcripts (GPA contribution or no). I was a bit pissed off on my AMCAS app because I had English Comp 1&2 from AP Language & Literature courses. It was listed as ENC1001 and ENC1002 on my college transcripts. AMCAS changed it to ENC1002 2xs. So I have 6 credits of ENC1002 on my transcript. However I took a mandatory writing course at my university as well as a British literature course, so I'm hoping that will count. I have no idea why AMCAS changed it.

Anyway, as long as English credits show on the transcript, I wouldn't worry about it.

English AP shouldn't be a problem. Obviously, science AP could be a little more iffy, but you should be fine without taking 2 semesters of English.

Even if a class doesn't end up fulfilling English requirements, we can take an English class(es) before matriculation, right? Although preferred, they don't have to be complete at the time of application?

Also, how would some schools know whether a course could sub for an English pre-req? For ex, taking a writing course under another department's heading which fulfills the university's writing requirement... how would the med schools know that this course counts towards the pre-reqs?
