So the dose for epi in neonatal resuscitation is 0.01 to 0.03 mg/kg IV as published in the AHA 2010 guidelines.

However, I read people say that you give 1-3 cc of the 1:10,000epi. This doesn't make any sense because 1-3cc of the 1:10,000 epi means you'll be giving 100-300mcg, right?

For 3.5kg neonate you want 0.035-0.1 mg, which equals 3.5-10mcg, right?

What am I missing?

Isn't it 0.1-0.3 mg of 1:1000? I haven't looked at it but I think this is your issue.

do you go by AHA guidelines or do you go by what you "read people say"?

and where did you get 30ug/kg?

you are missing PALS - last time i checked, it was 10ug/kg - but those are guidelines, and you are (i assume) not a nurse or a technician, so give the right dose...

you are correct. there is an apparent discrepancy between the actual guidelines (10-30ug/kg) and the published PALS algorithms (10ug/kg), which interestingly enough also come from the AHA.
 
my bad - i offer a partial apology as your information was partially correct - not given in per kilo dosing but correct for a 10kg child per AHA guidelines but not correct per pals algorithms
