ERAS activities question

Quick q: my school is having us fill out a form in anticipation of ERAS to get our stuff in order and I had a question about activities. I can code and one thing I've done since undergrad is help teach this to others for free. I think it's a great skill to have moving forward for these kids. In Med school I did this with 2 kids.

Am I going to need to list any contact info for this though? I can give the dad's name but they literally moved out of the country last year and I have no way to reach them any more to ask for an email address or anything. If needed I can give contact info for people I taught in undergrad because some of that's still in my phone, but that's all I got.

I didn't list contacts for many things. Didn't include hours for some things. I don't think they really sweat the details that much, they're really trying to get a general picture of the applicant
 
Same as above. Contacts aren't important, and you can mention hours in the activity description.
