I assume the only reason you would put things from undergrad is if you have nothing from medical school and need something there or it is truly significant. Otherwise, it should be volunteering from medical school.
I put college volunteer experiences on ERAS and never had anyone say it was odd or ask me why. In fact, they were major points of conversation in many of my interviews. If you think they're worthwhile, I would include them.
