If a pt's family cannot decide what to do with a pt who cannot make medical decisions for him/herself is the best answer to go with the highest surrogate decision maker (parenta vs adult chilsren, ext.) or refer to hospital ethics committee. i've seen both answers (and this is why you shouldnt test ethics on a standardized exam...).
