Some nitpicky questions:

If you're at a pre-interview dinner or interview and you are talking with a resident whom you know on a first-name basis, do you still recommend addressing the person as "Dr. LastName" simply for the formality of the event, especially when talking with the resident in front of other candidates who may not know the resident as personally as you?

Some of the programs who have invited me for an interview have only given me one available date. I'm trying to schedule interviews by location, and I'm also trying to avoid scheduling close to one of my fourth-year clerkship exam dates. Am I asking too much if I inquire about other interview dates, especially with the prestigious programs that are often sought after? I don't want to make it look like I'm not interested in the program by asking for other dates. I just want to attempt to schedule interviews in somewhat of an organized fashion.
 
As far as what to call the residents, most will introduce themselves as Bill or Jeff, not Dr. Smith.

If you know someone, you should just call them by their first name. None will care, as long as you are not calling them Jackass outloud (even if you do at other times).

As far as interview schedules, most programs will understand your situation and try to work with you. Remember, just like you want them, they want you. They just don't need you like you need them.
