Evaluating med school--anyone have a form?

Since I am clustering schools, I think I really need to spend time after each interview filling out an evaluation form. Does anyone know of a book I could photocopy one from? Or what would be the standard questions I need to ask myself? I guess I'm not sure how much to weigh things like faculty, curriculum, vs. gut feeling.
