evaluation forms with LOR

I had my letters of rec sent to the schools already but I didn't realize all the schools have there own evaluation forms to be completed with LOR's. Can a LOR substitute for a evaluation form or do schools usually require both?

It depends on the school, some say can replace, some say can send both.
 
I never sent the forms that the schools had. Your LOR writer would be doing a whole lot of writting if they had to do a new form for every school. My letters were all on the form that my pre-med committee gave out. This was used for both DO as well as MD applications.

I did the same thing. some schools, NSU, has a pretty long form, what doctor, or prof has time to sit an fill this out. I wouldnt DARE ask the doc i shadowed to do that, it would be too disrespectful.
I would love to know who makes up some of the schools policies.

Can someone start a thread of all the annoying things that medical schools make you do, PLEASE.
 
I didn't send them. They usually say they're optional. They're just a cheat sheet in case someone doesn't know how to write an LOR.
