Evaluations for ADEA PASS

Hey guys I was just wondering if anyone knows what type of evaluation PASS wants from our evaluators. In the past I heard it was a question and answer prompt style but I called and it seems that they have to write an actual letter now. I am asking so that I know what to tell my evaluator in in terms of what to expect and have ready come may. Thanks!

It has always been letters of rec but when they go to submit the letter there are also questions from what I understand.
