If my professor who's going to write my LOR is not a PhD, then her title should be Ms or Mrs correct? Not Dr.

Pretty much.

And if you do put the wrong title, then they can always change it. I put my science teachers as a Dr. So-and-so, and she ended up switching it on AADSAS to Mrs.

That was a lil bit embarrassing on my part. Woops
