For all those who claim that pathology job market is 'OK'... You sirs are misinformed...

All the listed positions for derm. It is well known that derm-trained pathologists have no problem i securing the position they dream of.

The question is about non-derm trained pathologists.....

Yes, derm has protected their specialty very well. Limited number of spots and highly competitive as we all know. Definitely one of the fields with the best job market. I remember asking a derm attending why the limited number of spots. His reply (cant remember his full explanation) was something like they didn't want grads to go out there and do cosmetics.

I've always wondered how many of those wanting derm would do it if the field was like pathology where you may have to relocate to even get a job.

Given the structural dissimilarities between pathology and most specialties, for a similar situation to occur in our field would likely require one of two scenarios:

1. Large numbers of practices that can barely keep up with their volume (the "restrict supply" approach).

2. Opening vast new revenue streams (a.k.a. "how radiology did it").

Option 2 seems far more palatable, but option 1 is the only approach I see put forth on this board.

Pretty much. It has yet to be seen whether most molecular pathology is worth embracing, but right now it's our only sizable opportunity for new growth.

Well if you had a dermpath or GU path fellowship I would bet your job offers would be similar with pod/reference lab offerings.

But having said that it is true that the some of the professional societies in particular for the surgical subspecialties go to great lengths to limit the number of residency spots. While in path the only limiting factor is CMS and the ACGME.

In part this is because path is dominated by academics and increasing residency spots doesn't harm the academic path market that much. Path is an inherently academic specialty while ortho is inherently non-academic. I mean the most famous orthopedic surgeons (i.e. operate on professional athletes are community practice ones). Therefore the relationship between community practice pathology and academic pathology is most acrimonious in medicine.

The cost of these molecular tests are quite expensive and the reimbursement not so much. You really need to have high enough volume in order to bring these tests in house. I don't know how many community practice groups have molecular testing but I don't think it would be much. So possible revenue stream from molecular testing would only benefit larger centers. I don't know if this is accurate but this is what seems to be true from what I've been seeing.

PCR used to be exotic, now you can run one for about $6 in the back of a Pinto. Technology is like an aging hooker - cheaper and easier with each passing year (and possibly done in the back of a Pinto).

And yes, molecular testing is being performed in community hospitals, mainly in the microbiology labs (think hookers again).

While it looks like there are a lot of openings for dermatologists, there are very few that I'd consider to be desirable locations. If you just look at the positions in CA, none of them are in SoCal & only 1 is in the Bay Area (Oakland).

In my opinion, a good job market is where I have no problems finding a position where I want to live. An ideal market is where I'm recruited for & offered a job while I'm still in training (where I want to live).
