Besides NBEO boards and state licensure (625 Part II + $625 Part III + $450 TMOD + $70 State law), what other expenses will I be responsible for near the end of 4th year or at least before I begin practicing. I am trying to see how much more money I am going to have to borrow to be able to survive before I start working. I am sure there are other misc fees that I am not aware of or just not thinking of...any help?
 
your controlled subtance licensure, DEA license, and any ancillary licenses if you plan on working at more than 1 location.

the DEA license is fairly expensive.
 
I remember spending $275 for my state license application fee (California), ~$15 copy of my transcript, ~$70 for my LIVE SCAN fingerprint service (fee varies depending on where you go), and then $374 for malpractice insurance. I'm sure there were other things but that's off the top of my head.

I didn't apply for a DEA number, but my friend who works at a VA said it was $551 for 3 years (which the VA paid for).

Also, what is the transcript for (just for my curiosity)? Do employers actually ask for it?

Last question....Has anyone worked under an optometrist's licensure after graduation, but before your licensure was complete? If so, can anyone give me an idea of what would be a reasonable hourly rate (i.e. or a percentage of doctor's pay)?

You have to check each state boards website to see what the application requirements are.

The list of state boards is here http://www.aoa.org/x5718.xml. You need your AOA user/pass to get in. Or just google it.

The transcript is usually required by the state board as part of the application (to prove you went to, and graduated from an optometry school). Again check the state boards

Again, I think this is state specific.

Not sure about that one, just make sure your malpractice is covering you specifically, not just the place you work at.

I believe this is illegal...


I'm sure other people will have some input here too.. but I think the best way to get the most accurate information is to check the state board websites of the states you are interested in, and then just call them to get any other questions answered. The few I have dealt with so far have been really helpful.

I did a bit of research (some free time since I'm recovering from laryngitis), and this is what I found for Missouri.

Missouri requires two sets of fingerprints for a background check. If you want to do it digitally, you go through Identix (contact info on the application) which is $52 (plus any other processing fee). Digitally takes 2-3 weeks for processing. If you want to submit paper and ink fingerprints, you need to use the fingerprint cards through the Board and that cost $39.25 (3-4 weeks to process).

An official transcript is needed for the application. I interviewed at two places and neither asked for my transcript.

Unless it's different in Missouri than California, it's illegal for you to practice without your state license. Take the free time to visit the local societies and start inquiring who is looking for an associate and/or partner and what the typical per diem rate is for your area.
