Experiences Section: Contact Info for Supervisor

I am not entirely sure what to do for the experiences section of the AACOMAS. What is the expectation when listing contact information for:

1. Physicians shadowed
AND
2. Supervisors at jobs or volunteer experiences where the person has moved on and I haven't been in touch with them

Any advice on this topic would be appreciated. Thanks.

I just put email addresses. If I didn't have their contact info, I put that they could not be contacted. However, I wouldn't do that for more than a couple of them.

I would totally call the help line that is linked to the APP. I called 3 or 4 times if I had a question and they were always extremely helpful!

For physicians I shadowed, if I did not have an email or number I was able to provide I just listed the general number for the emergency department or hospital. I mean, technically it's not incorrect info and realistically it should be a pain enough for whomever actually wants to call that they probably will give up after a few minutes lol.

Same reasoning for supervisors at jobs or volunteer experiences. I would put the last known number and let them deal with it. The company may be able to direct them to wherever they might be working at currently.

Put the doctor's clinic phone number.

Put whoever the current supervisor is or the general phone number of the department.
