Speaking for OMFS externships:
Just about every OMFS residency can have an externship. Some externships are more formal, advertised and elaborate than others, but every residency can have one if you call and see if you may apply to it.
Key thing is call and ask, some residencies have very informative departmental websites, some even with externship application that you can download. Be active and look around.
About not having anymore time off to do externships, well, you have to make time if you wish to participate in an externship. You have to apply with your dental school to give you time off of school for you to participate in externships. Most students only do externships during holiday breaks (one week during spring break and two weeks during winter break, etc.). If you wish to participate in a longer period of time, then you must ask to be excused from classes and/or clinic.
Example:
At NSU Dental, for any "off campus" (externships) rotation that you wish to participate in outside of vacation time, we must complete and fill out an "off campus" rotation application stating dates of participation, who, when, and where.....
Once the form is completed, then two signature must be obtained from:
1) Pre-doc Clinic Director - to verify that you as the student are not behind in clinic because getting your DMD degree is first and foremost. If you're behind in your procedural requirements, then you are not allowed to leave campus
2) Dean of Academic Affairs - to verify that you're a student in good standing, you're at least top 50% of your class, and no failures are on your transcript. Further to provide you malpractice/liability insurance during your "off campus" rotation participation
Once those two signatures are obtain, our externship participation is approved by the dental school, this does not mean you are accepted to the externship by the externship residency (some externship are very competitive but some are freebies where they allow anyone to participate). Externships has their own participant requirements.
When I participated in a month long externship this past summer, I had to apply for 3 weeks off of mandatory clinic days. I missed 3 weeks of scheduled clinic days. One week summer vacation, 3 weeks excused, total to one month participation.
Currently, I'm doing a 3-week externship where two weeks are winter/holiday vacation and I had to apply for 1 week to be excused from clinic. I'll be missing the first week of school for Winter semester to total up to my 3-week externship.
As you can see, you have to make time to participate in externship if you wish to do one beyond vacation time or make time period if there are no vacation time available. You also have to be sure that you're squared away at your dental school first so they let you go and excuse you from classes and /or clinic. You also have to be sure that you are accepted to the externship that you wish to participate in. A lot of things need to be done before one can participate.
This is just my experience. Every externships that I have applied to and have or am participating has very formal application process.
