If I'm shooting for a competitve specialty such as dermatology, how important is it to be involved in leadership activities, community service, etc.? Or is it mostly about letters of rec, Step I and research?

Not really important at all. Having absolutely nothing would obviously look bad, but you should have at least a handful of Leadership/EC/Volunteer. Doesn't have to be extravagant or crazy like in college.

Program directors in competitive specialties can't really compare extracurriculars between applicants, so unless it's something that is of national importance (started a national melanoma march, designed a free online course about sun protection, etc.), it doesn't really matter. ECs probably play the most weight during your interview if you can come up with interesting/memorable stories to share.
 
Do something if it wont cost you big on boards or on doing well in class. Are you at a P/F HP/P/F or A/B/C/D/F school?
