I am Canadian citizen attending American Medical School. I have not yet applied for OPT.
But I heard that OPT can take longer than the 90days that it says on the website. Have you heard of people not receiving the OPT in-time? If so, when did they apply that they were delayed?

What happens if I do not receive the OPT in time to start residency program for orientation in June? Will the program let me start a bit later or will they 'nullify' my contract?

So far, I have never had a delayed OPT.

I don't know if you can apply now, or if you have to wait until you know where you are going. If you can apply now, you might as well do so. You should talk to the visa officer at your medical school, the OPT is part of your F visa so they will need to help you with this.

If you are not ready to start on July 1, programs can wait for you, or they can nullify your contract. It's their choice. Most programs will wait a short while, but it totally depends. I often schedule interns on visas with a non-call first block, just in case.
