Is this correct?

increasing temperature increases reaction rates of BOTH forward and reverse reactions

increasing reactant concentration increases rate of forward reaction, but not reverse reaction

increasing product concentration increases rate of reverse reaction, but not forward reaction
 
The only thing you can say for sure is the effect of temperature - which you have correct.

Without knowing the order of the reaction with respect to each reactant - one cannot state the concentration's effect on rate.

Say we know that the forward reaction does depend on the concentrations of each reactant, and same for the reverse reaction. Then increasing the reactant concentration will increase the rate of the forward reaction... but can we conclude anything about the rate of the reverse reaction?
 
Not without knowing the rate law for the reverse reaction or even if the reaction is reversible.
