I am applying to medical school this cycle, and am wondering if I should update the medical schools I applied to with my fall semester grades. For context, I got a 4.0 this semester, but my GPA did not move significantly (3.92 -> 3.93 and sGPA 3.88 -> 3.90). Additionally, can I just send the unofficial transcript?

Frankly, I wouldn't waste anyone's time with that kind of an update. It will not move the needle. No one is waiting to see if you continue to do well in your coursework when you have done very well and it will not change the numbers displayed in your AMCAS application.

Yes, your grades are awesome. You don't need to update us with this fact. If you haven't heard back from schools regarding interviews, believe us that your GPA is not the problem. Do what you have to do, but don't make this the only topic of an update.
