Sacrix and the MDT product are basically big screws across the outer ileum and through the joint.

there are about 1000 bone graft wedge type products to choose froM.

I would not try to blend these two groups as while they use the same cpt are clearly different procedures.

I have done maybe 15 or so cornerlocs. Maybe a few more but I think less than 20. Only 3 have not had much improvement.

Where is the 12 month data on these patients? I’ll bet you $100, that none of these devices have a randomized, placebo controlled, trial to 1 yr demonstrating >50% relief of pain.
 
Which of these procedures has something removable in case it makes them worse?
 
Cornerloc certainly could not be removed. Sacrix and MDT Rialto could probably be removed but the trauma and the huge holes in the bone after would make that something that should be avoided.
 
I could be wrong, but it is totally in drusso's wheelhouse to post about sij fusions, knowing full well that we would question the practice. Then he could pivot to more Centeno propaganda. It looks like this was a 2 step brainwashing. Taking it to another level.

At least it isnt directly anti SOS stuff

Weren't you ever on the debate team in high school? You pick a topic out a hat. The teacher flips a coin: Heads = pro; Tail = con. Go. If we don't debate controversial **** what are we going to talk about?

Everything old is new again. It is like PRP- that failed 25 years ago; so most of the guys who know it doesn't work are retired, so it is time to bring it out to a new generation of physicians.

SI fusion was popular in the mid 90s. What happened? ALL of those patients developed severe degeneration of their hips (requiring arthroplasty) and had this weird "gangsta rappa" gait. It is physics- those forces have to go somewhere when the SI joints are fused and they go laterally.

Like PRP, SI fusions failed in the past and they are failing again. However, never let past experience and logic get in the way of a good paying procedure.

The hardware they were using then was essentially the same as lumbar rods and screws, requiring a large incision through the gluteal muscles, right? Isn’t it possible that the disruption of the muscle is what led to the gait issues, and a less invasive technique may produce superior results?

Fusion then was perc screws x2 in each joint. Never worked then, no reason it will work now. Great that every company has a kit to sell and training is going on all over the country.
 
SIJ fusion is one of the worst procedures I can think of and I would never do this to a patient. Inflammatory sacroilitis responds to meds/steroid injection, OA of SIJ responds to steroid injection.

SIJ laxity can be treated with good PT and possibly prolotherapy/PRP.

SIJ dysfunction/laxity needs good PT and many docs don't understand what isn't good PT of the SIJ. Seriously 2/3 PTs are not that good at SIJ dysfunction. You have to find a local PT with advanced post grad training to get high quality SIJ PT.

If all of the above fail, then SIJ ablation is indicated, preferably cooled RF, but bipolar, or strip lesion with 16G cannula also helpful. But never ever an SIJ fusion.

He’s talking about a C walk. 107 Hoova’s used to run my hometown back in the late 90s. Great description.
