Fellowship abroad?

I am familiar with several prominent ophthalmologists (G Tabin for one) and other surgeons (neurosurgeon Ben Carson) who have completed fellowship training at highly prestigious institutions abroad. Both of these individuals did this in the 80s.

Anyone know how this works now? Have requirements changed? Is this still a possibility without having to jump through hoops when coming back? General thoughts?

Two reasons to seek a fellowship abroad: the chance to work at an extraordinary institution and the chance to work with an extraordinary mentor at an acceptable institution. Same goes for domestic fellowships, for that matter.
