So I failed step 2 ck, but I was able to match at a small community program this year.

Is it even possible to match into fellowships with procedures in them like cards, GI, pulm/CCM? I am assuming no, but I want to check to make sure before I start making decisions.

Of course I plan on being the best resident possible, as my patients deserve no less.

If in house you might have a shot.

Community program might make cards/ gi tough on its own right

Coming from a small community program makes it hard enough for GI and cards, failing a step is basically a screen out. Sorry. I’d echo the above that you’d basically need to start gunning for that home program but even that is a stretch.

Thanks everyone for their honesty. What about CCM?

Plain CCM is not as competitive as Pulm/CCM but no matter what your step 2 failure will always be a red flag. Also there are not that many pure CCM programs for medicine applicants from what I’ve heard - a lot more are being geared towards EM and anesthesia folks. This is not saying anything against their qualification, just that this is much more common.

Your best bet is in house or another program within your city etc. Pulm more likely than Cards or GI. Finding the right faculty mentor who can advocate your case is key.

If OP went to a small community program, it is less likely program has in house fellowships.
If you really want a competetive specialty like cards/GI/pulmcrit, try to see if you can build connections with your subspecialist attendings, they may be able to make phone calls to PD's for you. And off course try to get involved in research/publications in the field you are interested in applying to. Otherwise if you really want to specialize, there's always ID/nephro/rheum/endo etc.

Sorry, but you have basically no shot. Almost all competitive fellowships use failed step exams as an automatic screen. Your application would never get past their censors. There’s a guy in the cards forum who has step scores of 230-240, over 20 publications, 2 years of dedicated cardiology research at big university hospitals, had LOR from big name players in the field with connections, but failed one of his step exams. He has not had a single interview for 3 years running.

Either plan on being a hospitalist or, like others said, aim for a non competitive subspecialty like nephrology or ID

Edit: link to the guy’s thread...

Need advice regarding Cardiology Fellowship

I would agree. I didn't realize Step scores would hold so much weight after residency but as I'm interviewing for a competitive fellowship right now, Step scores are brought up during the interview at almost every program.
