It means some program suckered you into doing attending work for fellow pay.

Not disagreeing with some of the comments above. But, fellowship is usually a good time to have some protected research and focused mentoring to prepare you for an academic medicine career. Probably not necessary for clinical track faculty, but a reasonable use of time for research tenure track faculty.
