I've been researching fellowships but all the ones I've come across seem to require completion of an intern year. Does anyone know any good fellowships for med students between 3rd and 4th year - that is, programs that provide good publication opportunities and/or put one in a good position to match at the program?

Look here for a potential funding source. As far as projects to work on, I'd start by checking out research being conducted in the department/division of your particular interest at your school.
