Doesn't particularly matter. You have the option at the top to set it to whatever exam you plan on taking/studying for. Choosing DO or MD will just make the default exam set to COMLEX or USMLE respectively.

Makes no difference. If you choose COMLEX, you still have the option of setting it for USMLE step 1, but if you select the option for COMLEX, you get sections on OMM as well, which are very valuable. As far as I can see, the sections on basic sciences are identical between USMLE and COMLEX.
