first aid says Cranial Nerve 3 ( oculomotor nerve) is only a motor nerve, but should it not be mixed because it carries parasympathetic fibers (sensory fibers too)...

Motor only - GSE, GVE. What sensory innervation does it provide?

Does ANS system relay in the sensory cortex or does it go only to thalamus??
Cerebral cortex (the insular and medial prefrontal regions), amygdala, stria terminalis, hypothalamus, and brainstem centers (periaqueductal gray, parabrachial pons, nucleus of the tractus solitarius, and intermediate reticular zone of the medulla).

Same here, but also 4.. Why don't you use 4? Because it is in the midbrain too?
IV is not that useful since it comes out dorsally (then circles around to go ventrally) and it is the only CN which crosses (similar to CST).
While, 3, 6 and 12 not only tells you the level of the lesion (Midbrain, Pons or Medulla) but they also tells you that the lesion is in medial portion of the Brainstem.

Ah I see. I've never had a question that tried to trick me by using 4 as an indicator of a lateral lesion (or anything other than a medial lesion). Good point though!
