if a program started to interview their 1st batch of residents, they asked " how do you manage the situation if you're one of our 1st batch of residents?"
How do I answer?

There would be dinner, breakfast and lunch with the PD and faculty (b'cos no residents there), what should I talk with them?

Should I bring my spouse to the dinner?

Do you want to be at a program that's just starting out? You should answer that question for yourself before your interview.

Probably a lot of the same things you'd talk with residents about in a similar setting (your interests, their interests, the program), but err on the side of being more careful about what you say.

Should I bring my spouse to the dinner?
Unless you're specifically told that spouses are invited, don't bring your spouse. Remember, food and drinks cost money, even if you're not personally paying for it.
