So I'm about to finish my first year studying medicine in Ireland and I have a question for some. What is the best thing to do that first summer? I have an opportunity to shadow doctors, should I take that? or should I get a job? study for USMLEs? I'm an American that wants to practice in America and I would shadow a doctor in my home town.

I did a preceptorship after my first year, and it didn't help me in any way.
I would recommend doing either:
1) something fun
2) something that pays
3) something fun that pays.
