Do they work either by primary repair or even arthroplasty/weil or fdl transfers? It seems that all non-op does not cure the symptoms? Another question is can a podiatrist harvest bone graft from proximal tibia in any state?

If the plantar plate is not too attenuated a primary repair can work. Sometimes adjunctiv procedures are necessary to correct any residual deformity of the toe.
If the plantar plate is attenuated and beyond repair typically a hammer toe results as well as a transverse plane deformity and a PIPJ fusion w/ FDL transfer and capsule work at the MPJ may be necessary.
I agree that non-op care just masks the symptoms of pain and once the steroids/NSAIDs are stopped and the toe splint comes off the toe will just start to drift again. I think it is a surgical diagnosis, but that does not seem to be the standard of care.
