The website and application suck. On the app, skip the part regarding the "by examination / by endorsement". I called MQA asking them what I was supposed to put, and they're like "I have no idea why that's on there". I think it really only applies if you pass your boards and then do internship hours so you can be an RPh.
The only consolation is that the license doesn't cost anything, other than getting a passport sized picture. Takes about a month, and then you get a nifty cardstock-ish license you can then hang on your wall.
