Focal neurologic signs = focal abnormalities?

Is it safe to assume those two terms are the same? So basically focal abnormalities are localized lesion in the brain that can be seen using CT or MRI or any imaging tools, correct? That's what they mean by "focal?"

"General" neurological deficit = decreased level of consciousness, attention etc. Focal deficit does not mean focal abnormality, just a focal consequence.
