Folic Acid 4mg daily starting one month prior to conception. This is 10x more than the 0.4mg recommended for all women one month prior to conception.

...by the way...I diagnosed a patient with fetal anencephaly a few days ago for the first time in my short career. It was not pleasant, but the key is to first offer condolensces and reassure the patient that the recurrence rate is low (~2%) and with preventative folic acid, this risk is reduced even more (by about 60-70%).
