Gender Reassignment Surgery?

I was wondering, in the experience of the residents and attendings out there, who does gender reassignment surgery (plastics, urology, gyn, general...?), how are they trained (Is there a fellowship that covers GRS techniques? Are they self-taught? Do they find and learn from a GRS surgeon mentor from the few in the country? ), and what are their practices like (do they do just GRS, or GRS + whatever else they're trained in)?

Since it's so specific, it's hard to find information on and searching the archives didn't come up with anything. Thanks!
 
I believe in Brazil, plastic surgeons do it... those guys have gigantic volume of patients... don't ask me why... not sure about anywhere else...

the only ones i've seen were on adolescents and they were done by a pediatric urologist.
 
Its usually a coordination between Uro and PRS...especially the male to female. AFAIK, there are very few places in the country which train people in this type of surgery.

Yeah, I have to doubt that this type of operation was done on teens. I would be very surprised. On Dr. Alter's site it says he trained in urology and then plastics...it's written as if he did 2 (competitive) residencies, which seems unlikely to me...I know there's a plastics/reconstruction fellowship off of urology so maybe that's what it's referring to.
 
Actually it is done one teens, but its a long process to get it done. First the parent(s) would have to agree to it and then the teen would have to go to two therapist (PhD, MD, or MSW) for 1-2 years. If the two therapist sign off that the teen understands want he or she is about to do, then the doctor can perform the surgery.
 
I know...I think sometimes I chose the wrong line of work.😳

If you are doing what you love the most, then you are not mistaken... besides that, putting and taking out silicone and botox is not much of an accomplishment even if gives you tons of money... 👍

to clarify - the one i saw on an adolescent was perhaps the second of surgeries (can't remember if she had one as a child initially). perhaps gender reassignment is misleading. i saw a child/adolescent with ambiguous genitalia who was raised as a female but with masculinized genitalia. so yes, pediatric urologist performed the surgery after much counselling and referrals to various other services.

Plastics and Uro. No way in hell I'd let an OB in on that action.


I think Gary Alter did a Urology residency and then did a PRS fellowship. Urology is an approved pathway for plastics. It's not very common but is possible.



It appears he did Urology at Baylor and plastics at Mayo. He also did a genitourinary reconstruction fellowship at Eastern Virginia which is very well respected for recon work. He seems like a fruitcake on the show but he is very well trained.

I was joking...I like what I do, but it seems like it would be nice to make enough money to have both a Beverly Hills and a Park Avenue practice.

Yep, that's what he did. He's BC in Both Uro and PRS.

No doubt about his training, although I wonder if he's had a little facial work himself. Something about his visage strikes me as odd.
 
I've met Alter at meetings a couple of times. He's a very gracious person. I don't think he does gender reassignment -- he's more into vaginal rejuvination. KC is right, he's BC'd in Uro and Plastics. He gives some pretty unique talks at our meetings because of his background.

And yes, his forehead doesn't move and he looks like he's had a deep plane face lift.

He might be "more into" vaginal rejuvenation, but here he lists it (gender reassignment) as a procedure he does: http://www.altermd.com and they did an episode of Dr. 90210 in which he went back to a program he'd done some training at (maybe in Colorado) where his mentor had had sexual reassignment surgery (revealed at the end of the show in a shocking plot twist!).

Yeah...I think he also looks like he's had a rhinoplasty and his veneers are too big for his face...unless you like that Gary Busey chiclet look.
