General Letter of Recommendation Question

I noticed that most IM medicine fellowship programs require

"Three letters of recommendations including one from your internal medicine residency program director."

Does the letter from an internal medicine program director count as one of the 3 letters or does this mean 4 letters of recommendation (ie 3 letters that are not program directors and 1 from a program director) ?

They require 3 letters, one of which should/must be from your IM PD. You can have up to 4 letters. One of which should/must be from your IM PD. You decide which way to approach it.
 
This is probably going to be a really stupid question, but I have two program directors willing to co-sign/write me department letters. Is there any utility to have two, or is it better to have just 1 and then fill the rest with other kinds of letters?
