Is it difficult to get a permanent position, especially in a large metropolitan area, after having done locums for a long time? I'm thinking like one year or more of locums. I get the impression that it has some stigma associated with it like you are a sub par anesthesiologist if you do it. Is that true?

A year of locums will not cause you any employment issues. If any employer won't hire you after doing locums, that is a clear sign, that the employer is out to screw you. That employer is looking for someone new to take advantage of, like someone straight out of residency.
