Goljan says that you first initially treat DKA with fluids, then insulin. This reminds me of a similar question in UW, which asks about DKA management, but the answer is insulin. I am not sure in I remember the UW well, though.

Any input?
 
Fluids then insulin is what I recall. Unless you can find and quote that UW explanation, I'll be sticking with this.

Maybe someone who's taken the step/is further along in their studies can comment?

You should start with fluids always even if blood glucose is >1000....one of my doctors once mentioned a case in which the insulin dose was dramatically decreased after they gave fluids and took a glucocheck again
 
I think UW is correct. We need to shut down ketone body production and hence insulin.

Actually, the first thing you'd want to do is to normalize volume.

Edit: I'm surprised this is within the scope of Step 1.
