How many programs should an average applicant apply to for PM&R residency? I ask this because I'm still undecided on neuro vs PM&R so I will most likely applying to both and it gets quite expensive after that 30th program. So far I have 20 programs, from the top places to average and about 7 premlim IM programs. I would like to stay in the midwest if possible. My stats:

DO student
USMLE step 1 - 225/step 2 - 230
Passed comlex level 1/2/PE
I have multiple letters (1 IM, 2 surgery, 1 neuro, 1 PM&R - pending, 1 FP)
Passed all rotations with honors except peds
Mix of volunteering and healthcare work experience
No research

Assuming you aren't going to ruin yourself during an audition rotation, go rotate at a big named institution that you may be interested in. It's not too late.

Get a good LOR from a well known attending in the field and you have a solid shot at any program in the country.

applying to programs is peanuts, compared to actually having to fly to places, transportation, hotel, etc. and you can turn down interviews once you get them.

estimating that you may get 60-80% invites for interview of the programs you apply to, you should strive to have at least 7-8 programs to rank. the more "backup" programs on your ROL may help you sleep at night during the month between rank and match.

I have done one rotation at Marionjoy and should be able to get a letter there. I'm doing a rotation at Schwab next month. I didn't have much time in my schedule and only was able to schedule 2 away PM&R rotation as I used the remaining 2 for neuro away rotations. I will try to get a letter next month and solidify my application.
