Need guidelines on what to prepare for.
This is what I am assuming.

1. I did a thesis project at my school. Can talk about it and answer questions about it in detail.
2. Basic science understanding questions. I am pretty good at it. SHould not have issues in this area.
3. Communication skills.

What else can I expect. Can they be from the following
1. probable research areas.
2. ROI on certain areas vs other areas.
3. Potential reseach problems to investigate into.
4. Problem solving done during my research

Of all the ones I would think only basic science understanding questions could be used to filter out people assuming that everybody's communication is equal.

Your thoughts please.

Why the MD, why the PhD; if you had to pick one which would you pick, possibly presenting your research and answering questions, any number of normal interview questions; a challenge you faced, how you got here, why this program, career goals, etc.

Depends on the school but you may or may not have a separate MD interview or an MMI so you can get ready accordingly.
