Grad School Application - Volunteer Hours

Soon I'll be finishing my volunteer time at one physical therapy place. I have all my volunteer hours recorded, as in what days I stayed and how long I was there. But what do I do now so that I can prove this on my application to grad school? I've asked my advisors if there are any official volunteer forms they want, and they said no. So is there any specific form that I should include with my grad school application or should I just make my own?
I appreciate any responses.

If you apply to any schools through PTCAS, they have a volunteer/shadowing sheet that you fill out through there and then the PT verifies it...

If not, just make something up in a word document and send it away!
