Grammar question about PS

Maybe this is a dumb question, but do you capitalize Internal Medicine or Radiology, or lowercase (internal medicine, radiology)?

I would capitalize it in terms of a course title, but not as a specialty. Meaning if you are talking about your rotation in Internal Medicine, I would probably capitalize it. But if you are saying that your dream is to enter one of the internal medicine subspecialties, you probably don't. I doubt this is going to make or break a PS though, as long as you are consistent.
