Would the average person be able to see that the patient had gynecomastia after surgery? And if so, what kinds of clues are there... for example large nipples? depression beneath the areola, etc...

Usually you don't get nipple-areolar complex hypertrophy (if you do you'd just do some kind of concentric "purse string" reduction of the NAC). There's usually not a whole lot of stigmata afterwards. A number of authorities recommend ultrasonic liposuction as the treatment with manual excision of the discoid shaped structure that tends to be under the NAC in these cases.
