What will you do when a 38 yo pregnant woman, 15 week gestation, comes to your office? Her 10 yo son has Down syndrome.
1)Propose amniocentesis
2)Tell her it’s very likely that the second child will have Down


Which one is more correct?

Tell her the risk is high and offer non-invasive prenatal testing, then amnio to confirm if the result from NIPT is suggestive of a defect.. Advanced maternal age does increase the risk for chromosomal abnormalities, but more importantly it increases the risk for procedural complications (like miscarriage).
 
you would still do both, but if you were to choose I'd go with (1). You offer her testing. You don't just tell her she's at increased risk without giving her option to screen. Is this a real question or are you being hypothetical and made it up?
