The respectful thing to do would be to cancel the interview or interviews right now...

can I cancel them the day of? med schools aren't very respectful in getting back to us for months on end...many times they interview people for waitlist positions knowingly.
 
If you really want to stick it to them, I suggest the following:

Go to the interview, do exceptionally well and accept their offer a few weeks after. Work hard and be one of their top students. Have the school be proud to have you as a student. Then during your COMPLEX exam, just walk out, give everyone the finger and drop out. The school will be so embarrassed and you will lower their pass rates! This plan takes some commitment but the payoff in the end is well worth it.
