I have congenital hearing loss, I essentially have 50% of normal hearing ability, and I am wondering if this is relevant to my medical school application. I don't consider this as something that is a part of my core identity but it definitely does have at least a moderate impact on my life.

Is this information relevant to my application, and if so, where would it belong?

And to add to the above, it doesn't hold me back in life in any important way. I just have to be more diligent in conversation and deal with the occasional embarrassing misunderstanding of what was actually said.

every school has technical admission requirements as well as office of disability services (example links below). You will need to address this after admission in pre-matrictulation phase

Technical standards | MD Admissions | Stanford Medicine
3.5: Disability Accommodations | MD Program | Stanford Medicine

I don't think I have any issue complying with technical standards. I haven't discussed it at an any job so far and I haven't had major issues. Does hearing loss have any relevance to diversity / adversity prompts?

I've had students with profound hearing loss. You will probably need some type of accommodation after matriculation, but that's about it.
