Do all of the prerequisites have to be completed during the time which you apply to dental school? Because If I plan on applying during Summer 2013, I will still have statistics and physics lab remaining, which I will finish up during Fall 2013. If I do apply, without having those 2 prerequisites complete before the summer, will it hinder my application/outlook on getting an interview or acceptance?

Another question that I have for those of you have been there and done it is:
How do you go about asking your science professors for LoR?
Do you present them with a sheet of paper that tells them what they need to complete? Or what do you do to make the process of obtaining a LoR easier?

-I plan to apply to Texas schools during Summer 2013 (after my junior year), or I can take a gap year and apply summer 2014 (after my senior year) and have a one year gap.

No, those prereqs do not need to be done in order to apply. However for your potential enrollment to stand, prereqs will need to be completed for the respective school. Conditional acceptances go out frequently with the understanding that the student will finish the required prereqs.

As far as letters go, just ask. Ask to set up a meeting with your professor, in person. Figure out what it is that needs to be in an LOR and any special requirements you might have. Have some sort of a resume with you detailing your additional credentials. They already know how you've performed in their classroom. Make your pitch on what else makes you a good applicant. Just don't be shy in this situation, it's asking a perfectly normal favor and chances are they've received such requests before.
